Pro Forma Invoicing Procedures & Policies

All orders require prepayment (we do not accept Purchase Orders). Some organizations require a Pro Forma Invoice that can provide verification and authentication of the exact amount of prepayment monies due. It should only be requested if your department requires it since Pro Forma Invoices do not qualify for coupon specials, other store sales, and free or reduced shipping offers. It is more cost effective to pay with a credit card (Visa, MasterCard, American Express or Discover Card) and order directly through the online store so you can take advantage of sales, specials, and shipping deals.

STEP 3: As soon as you receive your Pro Forma Invoice ...
- Present the Pro Forma Invoice to your finance department
- Send payment to us along with a copy of the Pro Forma Invoice number
STEP 4: Once we receive payment ...
- Your order is processed and posted according to our current shipping schedule
- You will receive a shipping confirmation email with the final Sales Receipt attached in pdf format
STEP 5: When you receive notification that your order has shipped ...
- Keep a close watch for your shipment
- Make sure you respond to any notices from Customs or the Postal Service regarding payment of any custom fees and local taxes
- The postal service in your country will not deliver the package until you pay whatever fees are due
- If you have not received your package or notice from Customs/Postal Service within the time frame stated in your shipping confirmation email, notify us immediately

If you are concerned about security, please note that we use Secure Sockets Layer (SSL) to protect the information you submit. When placing an order with Cognitive Therapy Training on Disc ™ please be assured all data is encrypted and your confidential information is protected. Our on-line store has extended site verification and authentication by Go Daddy Secure Certificate Authority validating that we use 256 bit SSL for secure transactions. Also, we do not store any credit card information on our computers.
During checkout, make sure you see the “https://” prefix in your browser bar, indicating that a secure session is under way.
Check Payments: the monies must be in US dollars drawn on a US branch bank. The name, address and complete AB routing numbers (9 digits) of the US bank must be imprinted on the check. Complete instructions regarding payment method and submission procedures are included on the Pro Forma Invoice Request form.
EFT/wire transfer: Must be in US dollars. All information regarding payment through EFT / wire transfer is provided on the Pro Forma Invoice.
Cognitive Therapy Training on Disc™ is copyrighted material. The purchaser is licensed to use the product for personal use. Listening and viewing is restricted to mental health professionals and students in a mental health field.
Permissions and Use Restrictions
Our copyrighted training discs are for use only by qualified mental health professionals or graduate students in a mental health program. Please make sure that use is limited to these groups. Any duplication, broadcast, other distribution, or electronic storage of this copyrighted material is strictly forbidden. Thank you for your attention and adherence to these permissions and use restrictions.
Eligibility to Purchase
Cognitive Therapy Training on Disc™ is available only to Mental Health Professionals or graduate students in a mental health field. Because of the need to qualify our buyer / end user, we are unable to sell our products to bookstores and other intermediaries. If you are ordering on behalf of your library, department, institution, etc., you need to provide the name, degree, affiliation, and email of the mental health professional responsible for the order.
We are happy to work with libraries but they need to provide mental health qualifying information and ensure distribution is limited to only the approved groups. Libraries wishing to purchase Cognitive Therapy Training on Disc™ must provide mental health qualifying information and agree to adhere to our "Permissions and Use Restrictions. "
Orders shipped to California Address
We charge the mandatory 7.75% California Sales Tax. This tax is not charged to orders that ship outside California (2016 rate was 8.00%).
Customs and Other Taxes
For orders shipped outside California, we do not charge for any taxes. Customs duties, local taxes, and any additional local fees are not covered by our shipping and handling charges. They are the sole responsibility of the customer. If any are due, you pay on your end. The amount and process varies from country to country. Please check with your customs agency for details. Please note: we are not responsible for delays, damage, or loss while in the possession of customs agencies. However, in our experience, problems are extremely rare.

We send email notification when an order has shipped. Once you receive notice, keep a close watch for your package or notice from your post office. All orders ship through the United States Postal Service (USPS). Tracking numbers are not available unless your order has shipped Priority Mail.
We ship orders every Tuesday and Friday but there are some exceptions. For CURRENT shipping dates see our Shipping & Taxes page.
USA Addresses: ship USPS First Class Mail. The estimated delivery time varies from 2 - 3 business days. Tracking is not available on First Class Mail. If shipping weight is greater than 13 ounces (more than 12 individual discs), we ship USPS Priority Mail with a 2-3 average business day delivery. You will receive an email from USPS.com with your Track and Confirm status. We ship all CATPAK and CDPAK orders via USPS Priority Mail.
Foreign Addresses: ship First Class Mail International (AIR MAIL). Estimated delivery time: 10+ business days. Tracking is not available. We ship all CATPAK and CDPAK orders via USPS Priority Mail International® with a 6 to 10 average business day delivery. You will receive an email from USPS.com with your "Track and Confirm" status. IMPORTANT REMINDER: You are responsible for any taxes (VAT, GST), Customs and postal fees assessed by your country. Your post office will not deliver until these fees are paid.
Incorrect Shipping Address
Please note that we are not responsible for orders with incorrect shipping addresses. If your package is returned to us and you would like re-shipment, you are responsible for return shipping charges.
If your package is returned to us as "unclaimed", there are two options available to you. (1) We can re-ship your package if you pay for new shipping charges and verify a valid Shipping Address. (2) We can issue you a refund minus the original shipping costs and a 5% re-stocking charge under the following conditions: the package has not been opened, the contents must not be damaged and the disc seals not opened. For refunds requests: all refund payment is in US dollars. If the order was by Visa or MasterCard, refund credit can only be applied to the credit card used for the original purchase. If the order was paid by check, we can only issue a refund after we have verified the department that was responsible for the original issuance. If we cannot make that determination, we will issue a store credit in the refund amount and send that credit to the email address of record. Store credits cannot be converted to cash and will expire 180 days after issuance.
Incorrect Products in Shipment
Carefully check your order when received to make certain your order is correct. If you were shipped an incorrect product, do not open the product. Replacement for incorrect products is only available if the products are unopened. Immediately notify us by email. Make sure you include your order # and a detailed description of the error. Save all packing materials & the shipping box until the issue is resolved. Notice must be sent within 10 days of Ship Date. We will work with you to determine the error and advise you as to what replacement/exchange action to take. If it is our error, we will supply the correct order with no charge to you. Do not return any incorrect products to us until you receive a Return Merchandise Authorization (RMA) number. Failure to follow these procedures invalidates the free exchange agreement.
All our Audio CDs and DVDs are professionally manufactured using high quality materials and processes. Any product found to have a manufacturing defect will be replaced free of charge for a period of 30 days from the date of purchase. Items that are returned to us without authorization do not qualify for warranty replacement. Manufacturing defects are defined as: disc will not launch or play and/or disc has no sound.
